Contributions must follow this process:

For line 4, Section A, the taxpayer ID number of Learning to Serve is 26-4196119.
  2. Georgia Department of Revenue will NO LONGER ACCEPT MAILED APPLICATIONS.   Contributors should sign and mail the completed Georgia Form IT-QEE-TP1 to:                    Learning to Serve, Inc.
    12460 Crabapple Road
    Suite 127

  3. Within 30 days, the contributor will receive confirmation from the Georgia Department of Revenue that the application for the tax credit has been accepted (or, if Georgia has reached its annual limit of $58 million in tax credits, denied).
  4. Within 60 days of receiving the approved Form IT-QEE-TP1 from the Georgia Department of Revenue, the contributor must make the contribution to Learning to Serve. To do this, the contributor must complete Learning to Serve’s questionnaire detailing where, if at all, the scholarship should be directed and mail the completed form, a copy of the approved Form IT-QEE-TP1 and contribution to:
    Learning to Serve, Inc.
    12460 Crabapple Road

  5. Learning to Serve will then send the contributor Form IT-QEE-SSO1 confirming the contribution. Form IT-QEE-SSO1 and Form IT-QEE-TP2 must be submitted with contributor’s Georgia state income tax return in order to claim the tax credit.
